On-The-Job Paid Meal Period Agreement
This is an agreement between ____________________________________________ (the employee)
and ___________________________________________ (the Company) in which the employee and
Company understand the nature of the employee's work prevents them from being relieved of all
duty during their daily 30 minute meal period.
The employee and Company agree to an "on duty" meal period which is to be counted as time
worked for purposes of calculating wages. This agreement remains in force until
cancelled in
writing by either party.
